the 2011 State Games of America.
PROOF
OF AGE
Please mail in your
your teams birth certificates
prior to the tournament in order
to speed up the process at Check-in.
All players must also present
a birth certificate copy at
check-in and have them
on site for each game. We will
not accept faxed birth certificates.
NOTE: A team
may have up to 3 players submit
an Age
Exception Form. Age
Exception Forms must be submitted
with a birth certificate and
last report card the player
received. A player may only
be one year older than the required
age for the division and be
in the same grade as the other
players. This form must be approved
by SGA prior to the
tournament for verification.
2011 AAU Basketball Grade Exception
Form may be submitted instead
of our Age Exception Form.
