1. Click on Registration Forms on the Left
2. Print & Complete Team Entry Form and Waiver
3. Check Softball box, write
in Division and complete Team Roster with
name, birthdates, & hometowns of each
player. You may attach a roster form or add
an additional page as needed.
4. Each coach must submit: Team Entry Form,
Team Entry Fee, & Waivers for each participant.

the age and level requirements. A copy of
birth certificate and/or player cards with
picture are required at team check-in.

roster to change from the year you qualified
for the State Games of America.

You must register at your CURRENT level
or age group. For example, if you qualified
as a 9 & under team in 2009, you may
be an 11 & under team for the 2011 State
Games of America.
