2. Print & Complete Team Entry Form and
Waiver
3. Write in division and check Water Polo
box. Complete Team Roster including Names,
Birthdates, and Hometown for each player.
4. Each coach must submit: Team Entry Form,
Team Entry Fee, and Waivers for Each Player.

Open
to any SGA Qualified team or CA team meeting
the age requirements.
You must register at your CURRENT level
or age group. For example, if you qualified
as a 14 & under team in 2009, you
may be an 16 & under team for the
2011 State Games of America.
